Every walk is different, so please complete a Risk Assessment (RA) for each one that you coordinate. This helps you plan and make a note of the steps you're taking to keep walkers safe. After a walk, Risk Assessments are stored for three years.
How to submit a Risk Assessment
There are 2 ways to submit a Risk Assessment:
Complete a simple online form (a few pages) - this is quick and easy, and can be done on mobile devices/phones.
Alternatively, if you prefer to fill in and edit a form using Word or even to print and take a photograph of a paper form, it can be uploaded as a document or file attachment. Blank copies listed at the end of this page.

If you wish to Upload a PDF, Word or other handwritten/scanned/photo of form (jpeg) , please remember to rename your document.
RA_yymmdd_eventname
Replace "yymmdd" with the event date, and "eventname" with the name of event; for example: RA_240531_walk_Lenzie_Moss.jpg = "walk on 31 May 2024 to lenzie moss". This will allow us to identify your form more easily.

Excel - to download and edit using Microsoft Excel.
*(will open in Google Docs in your browser - if you wish to download the file for editing on PC etc - then choose "⁝" from menubar, then select "download" - or on some devices "send a copy" )
